The Wisconsin Youth Health Transition Initiative and Wisconsin Leadership Education Neurodevelopmental and Related Disabilities (LEND) invite you to attend a live-stream web conferencing event of the Baylor College of Medicine’s 18th Annual Chronic Illness and Disability Conference: Transition from Pediatric to Adult-based Care.

Join these organizations as they host a two-day conference focused on health care transition, discussion, networking, as well as idea sharing among participants to gain knowledge and information about how best to plan for a successful transition to adulthood.

This event is focused on information that will be useful to physicians in internal medicine, family practice, pediatrics, and psychiatry; other health care providers including psychologists, social workers, nurses, dietitians, case managers, counselors, and primary care providers and anyone interested in moving transition efforts forward in their organization.
