Transition Learning Community

The Youth Health Transition Learning Community provides networking opportunities for health professionals who wish to move transition forward in their own organizations. This statewide group comes together virtually to share ideas and best practices around youth health transition. Health professionals of all types  are welcome--physicians, physician assistants, nurse practitioners, nurses, social workers, etc. The Youth Health Transition Initiative, based at the Waisman Center in Madison, facilitates Learning Community activities. 2017 Youth Health Transition QI Grantees:

  • Gundersen Health System - General Pediatrics 
  • University of Wisconsin Hospital and Clinics; Pediatric Primary Care

The aim of health care transition is to optimize health and assist youth in reaching their full potential. One primary goal of the Youth Health Transition Quality Improvement (QI) grants is to support practices and health care systems in initiating quality improvement processes that support youth health transition.

Join us for a webinar where the 2017 Youth Health Transition QI grantees reflect on their ongoing grant work as well as strategies they've implemented to establish health care transition within a practice setting. This is also a great opportunity for those interested in applying to our 2018 QI Grants to hear about the grant process, and ask questions of current grantees and their projects.
