Our Mission

For Healthcare Providers

The Youth Health Transition Initiative provides technical assistance to develop best practices in transition and policy in a clinic or health system. Healthcare providers can access information, resources, and  training.

For Families

The Youth Health Transition Initiative provides training and materials to the Regional Centers for Children and Youth with Special Health Care Needs so they may share with families. Families can access information and materials on our website and at conferences and gatherings focused on transition.

For Community Members

The Youth Health Transition Initiative provides training and resources about healthcare transition to our Partners. We encourage them to include healthcare in future planning with the youth they serve. Our staff participates in statewide initiatives, such as the WI Community on Transition and the WI Integrated Transition Planning Project. We also represent WI in national efforts, like the Got Transition® supported Healthcare Transition in Schools. The goal is to make sure that every youth can be as independent in their healthcare as possible as they transition to the adult system. Community Members can access information and materials on our website and at conferences and gatherings focused on transition.
