Transition from Pediatric to Adult-based Care

This livestreamed conference is about transition from pediatric to adult-based healthcare.
Attendees will be able to ask questions.
The purpose is to share knowledge and information about how to support individuals,
families, and clinicians to plan for a successful transition to adult healthcare.

WI residents:

• OR the Youth Health Transition Initiative will be able to provide a “scholarship” to cover the registration fee for some participants to attend. After August 30, you can request “scholarship” information by emailing Sonja Oetzel at or by calling 608-265-8955.
