Fall 2025 Edition
Transition has many pieces! Learn about what those pieces are and how you can stay a step ahead so every child can be successful. All Transition Talks Tuesdays (TTT) will be recorded and resource sheets will be made based on the recording. You can come back to this page to access the recordings.
The Wisconsin Youth Health Transition Initiative aims to make healthcare transition easier for youth with special healthcare needs in Wisconsin. We do this by developing and sharing healthcare transition resources with families and healthcare providers. We also work with healthcare providers to support best practice transition care for patients with disabilities and special healthcare needs.
Our Initiative is based out of the University Center for Excellence in Developmental Disabilities at the University of Wisconsin-Madison Waisman Center. We believe all families (including the self-advocate) deserve the best information possible so they can make their own decisions. TTT is one way we encourage transition to be integrated across all parts of life.
The Fall 2025 Edition is inviting some different partners to answer the question “how do you support adolescents who are moving into adulthood and their families?” and then tell us a bit more about their work. And to keep switching things up, we gather during the day — all sessions will be at noon.

What’s After High School?
September 23, 2025
What’s After High School offers families an overview of the transition to adult life for young adults with disabilities and special health care needs. The session is most helpful for families with children ages 14 – 21, and the educators and providers who support them. We’ll cover:
- Planning for the future and important dates for transition
- Supported Decision-Making & Guardianship
- Long-term support programs and health care changes
- Education planning including college/post-secondary education
- Living and working in the community

How do the Wisconsin Technical Colleges Support Youth Moving into Adulthood and their Families?
September 30, 2025
Join us for an in-depth look at how the Wisconsin Technical College System (WTCS) supports students with disabilities as they transition from high school to postsecondary education and beyond. This session will highlight the collaborative efforts of WTCS Disability Services, including inclusive practices, accommodations, and transition planning that promote equal access for individuals with disabilities.

Supporting Students with Disabilities in Transition to Adulthood
October 7, 2025
As Wisconsin’s Parent Training and Information Center, WI FACETS (www.wifacets.org) provides Wisconsin families with children with disabilities with personalized support to navigate their transition to adulthood. We offer 1:1 phone assistance on special education and Transition IEP-related questions, along with a wide range of self-advocacy resources—both online and in print—to empower youth to participate in their own decisions, understand guardianship, and explore supported decision-making options. Our goal is to ensure that families and students feel prepared, confident, and actively engaged in Transition IEP meetings and Postsecondary Transition Plans.

How does the Youth Health Transition Initiative Support Youth Moving into Adulthood and their Families?

As we look ahead to our students/children turning 18, we need to keep an eye on health care. What steps need to be taken to move into the world of adult health care? What skills can be taught (even in middle school)? How can we tell what a student needs to know? We welcome in Hunter Markle, self-advocate, and his dad, Tim Markle, Director of the Youth Health Transition Initiative.

How does the Wisconsin Statewide Parent Educator Initiative (WSPEI) Support Youth Moving into Adulthood and their Families?

WSPEI is all about partnerships, helping families and school districts to find resources that will help them build positive working relationships leading to share decision-making. We have resources dedicated to moving into adulthood. We welcome in Cheri Sylla, Statewide Family Engagement Coaching & Indicator 8 Coordinator.

How do the Wisconsin Independent Living Centers Support Youth Moving into Adulthood and their Families?
This session has been cancelled and we will look at rescheduling in January 2026. (Previously scheduled for 11/11)
Wisconsin’s Independent Living Centers are consumer-run organizations that provide an array of services to people of any type of disability, and of any age, including supported young adults moving into adulthood.

How does the Wisconsin Transition Improvement Grant Support Youth Moving into Adulthood and their Families?
November 18, 12:00-1:00pm
The Transition Improvement Grant (TIG) is designed to strengthen the transition process in our Wisconsin schools to increase high school graduation rates, decrease dropout rates, improve the quality of postsecondary transition plans (PTP), and develop strong career, college, and community readiness among our students with disabilities through positive post school outcomes.

How does Wisconsin’s Juvenile Justice System Support Youth Moving into Adulthood?
November 25, 12:00-1:00pm
The Division of Juvenile Corrections (DJC) works to reduce delinquent behavior and promote safety for victims and the community. The mission of DJC is to promote a juvenile justice system that balances protection of the community, youth accountability, and competency building for responsible and productive community living.

How does Wisconsin’s Foster Care System Support Youth Moving into Adulthood?
This session has been cancelled and we will look at rescheduling in January 2026. (Previously scheduled for 12/2).
Independent Living Services are required for youth ages 14+ that are placed in a foster home or another out of home care setting. These services help teach youth the basic life skills they need in order to become successful adults.
