The Youth Health Transition Initiative is funded by The Health Resources and Services Administration (HRSA) Maternal and Child Health Bureau (MCHB) Title V Maternal and Child Health Block Grant. Wisconsin has chosen to prioritize work on National Performance Measure 12, which is transition to adult health care for children ages 12-17 with and without special health care needs. The Youth Health Transition Initiative is housed at the University of WI Madison Waisman Center and acts as a hub of expertise for the state. Wisconsin chose transition in 2016 because only 19% of youth ages 12-18 were receiving adequate transition from pediatric to adult health care services. In addition, the number of healthcare systems and clinics having a transition policy was and still is very low.
